Assessment and Care Planning

Service specific assessment and care planning is a core component of a wellness approach.  It enables organisations to discover what clients, carers and families need in order to live as independently as possible and helps us to gain an understanding about what the client/carer hopes to achieve.  Service specific assessment and care planning involves developing and documenting the strategies that will support the client achieve their goals; prioritising needs and actions to optimise outcomes.
 
In this video, the Eastern Sector development team explore a good practice approach to assessment and care planning, that includes consideration of a person’s diversity characteristics.  The video also focus on the value of assessment and care planning as a tool for the client and provides some helpful tips for introducing care plans to clients;  identifying outcome focused goals and documenting good care plans.
