Initially established in 2010, regional sector support roles, now known as the Eastern Sector Development Team (ESDT), were implemented by the Victorian Department of Health and Human Services to support the Victorian Home and Community Care (HACC) program. The scope of these roles changed over time and currently we support local Commonwealth Home Support Programme (CHSP) in the eastern metropolitan region of Melbourne.  We work together to share good practice that promotes inclusive access and person centred care.

The key objectives of the ESDT are to:

  • create a shared understanding of the Wellness and Reablement approach and Diversity Planning and Practice initiatives
  • strengthen working relationships between local CHSP agencies
  • build the capacity of local agencies to embed key elements of quality service delivery

We meet with the regional sector bi-monthly and encourage the involvement of senior management, program leaders, clinicians and program staff from across the region.

In addition to the regional practice forums, we recommend providers build their capacity through other ESDT resources and collaborative opportunities promoted via the ESDT website and YouTube channel.

An overview of how the Eastern Sector Development Team can support you is available via this link or you can contact them directly on email address: esdt@each.com.au.

The ESDT celebrates the diversity of the EMR and actively supports our funded service providers to make their services inclusive and accessible for everyone.  We acknowledge Aboriginal and Torres Strait Islander people as the traditional owners of country throughout Australia and pay respect to elders past, present and emerging.

The ESDT is supported by the Australian Government Department of Health.
