The Eastern Sector Development Team (ESDT) support the implementation of inclusive aged care services, built on the principles of equity and person-centred practice. We deliver this through the promotion of responsive diversity, wellness and reablement approaches that focus on the strengths and abilities of people, their preferences, needs and aspirations. We are funded to work with Commonwealth Home Support Programme (CHSP) services in the eastern metropolitan region of Melbourne. 

  • We connect with local providers, create opportunities for regional partnerships and host forums, workshops and professional development activities.
  • We support local providers to deliver aged care services that embodies good diversity, wellness and reablement practice.
  • We promote, develop and share good practice that enables services to adopt progressive and innovative approaches to working with and supporting older adults.
