  • The Aged Care Reforms Communication Toolkit Release 1.0

With limited time before the new Act takes effect, providers must ensure their staff, clients and governing boards understand what’s changing and why it matters.

The ESDT and our Victorian SSD partners have engaged Enkindle Consulting to develop a practical and time-saving communications toolkit to support CHSP and Support at Home providers during the transition to the new Aged Care Act and Support at Home program.

This Communications Toolkit bridges the gap between government-issued materials and what providers need on the ground. It includes:
  • Editable templates to tailor communications to staff, clients and boards
  • Clear, audience-specific messaging to support understanding and reduce confusion
  • FAQs and staff scripts to help frontline teams respond confidently and accurately
  • Resources that promote compliance, minimise risk, and maintain trust across all stakeholder groups.

The toolkit ensures all key messages are delivered consistently and clearly, saving providers the time and pressure of creating materials from scratch.
The toolkit will be released in stages throughout June.

Associated Providers under the new Aged Care Act- New Regulatory Model

This video will help the providers to prepare for the changes coming to aged care from 1 July 2025.
From 1 July 2025, third party providers and subcontractors will be known as associated providers. They will deliver services on behalf of a registered provider
Registered providers are responsible for the quality of care delivered on their behalf and ensuring their associated providers comply with relevant obligations. Under the new Act, registered providers cannot contract out their regulatory responsibilities.

Workers of an associated provider will be considered to be workers of the registered provider for the purposes of the Aged Care Act. This means they will need to be screened to make sure they are suitable to work in an aged care setting.

An associated provider may become a registered provider in their own right.

Once the new Act commences, registered aged care providers will have to report details of current associated providers that are delivering aged care services on their behalf vis GPMS.

Would you like to be a registered aged care provider?

The Aged Care quality and Safety Commission (ACQSC) has released a Draft Audit Evidence Collection Tool (AECT) - Registration that helps the ACQSC to understand if the organisation or person can conform with the related strengthened Aged Care Quality Standards. 

The use of the tool is mandatory for organisations interested to be registered in categories 4,5 & 6. 


ACQSC use this tool to collect evidence about:
  • the systems and processes you will establish
  • how you will deliver high-quality care to older people.

Data Exchange Dictionary - Stage 1

The Data Exchange Dictionary (Data Dictionary) assists service providers to enter data into the Data Exchange (DEX) in a consistent way that best reflects the program activity being delivered. This document provides policy guidance on entering data into DEX for Commonwealth Home Support Program (CHSP) services funded by the Department of Health and Aged Care.

Please note that from the 1 July 2025 only Stage 1 fields will be available in DEX for service delivery:
• Hours
• Minutes
• Total Cost
• Quantity
• Fees Charged

In preparation for the 1 July 2025 data reporting, we suggest the following actions. Please note that some of the actions are time sensitive to ensure that you can claim funding for service delivery timely to support financial viability


𝗔𝗰𝘁𝗶𝗼𝗻𝘀:
• 𝗨𝗻𝗱𝗲𝗿𝘀𝘁𝗮𝗻𝗱 the new CHSP Service List
• You will 𝗻𝗼𝘁 𝗯𝗲 𝗮𝗯𝗹𝗲 𝘁𝗼 𝗰𝗹𝗮𝗶𝗺 for services provided post 1 July 2025 until you add new Program Activities in your DEX Portal
• New program activities will be available in DEX 𝗳𝗿𝗼𝗺 𝟭 𝗝𝘂𝗹𝘆 𝟮𝟬𝟮𝟱
• 𝗔𝗱𝗱 𝘁𝗵𝗲 𝗻𝗲𝘄 𝗣𝗿𝗼𝗴𝗿𝗮𝗺 𝗔𝗰𝘁𝗶𝘃𝗶𝘁𝗶𝗲𝘀 to your Outlet for Helpdesk to approve asap (𝗼𝗻 𝗼𝗿 𝗮𝗳𝘁𝗲𝗿 𝟭 𝗝𝘂𝗹𝘆 𝟮𝟬𝟮𝟱)

• 𝗖𝗿𝗲𝗮𝘁𝗲 𝗻𝗲𝘄 𝗰𝗮𝘀𝗲𝘀 to associate with the new Program Activities from the new Service List soon as Program Activities status is 'Active"
• You may need to 𝗹𝗶𝗻𝗸 𝗰𝗹𝗶𝗲𝗻𝘁 𝗿𝗲𝗰𝗼𝗿𝗱𝘀 𝘁𝗼 𝗗𝗘𝗫 in your client data management database (depending on your CDM design)
• Commence reporting from 1 July 2025 for approved services only

Statement of Rights - UPDATE

At the ‘New aged care regulatory model – provider obligations and rules’ webinar on the 8th of May 2025 providers were advised that they will need to get participants to sign the Statement of Rights from July 1.  This is contrary to all the documented guidance; therefore, SSD has sought clarification and have been advised that signing of the Statement of Rights is not a requirement of the new Act.

Under section 155, the rules prescribe that a provider must provide an older person information on their rights, a copy of a Statement of Rights, and assist the older person to understand the information and the Statement of Rights. 
 
A registered provider must comply to do this before, or when, the registered provider commences delivery of funded aged care services to that older person. 

Record keeping rules already require registered providers to retain records showing compliance with their obligations, including provision of the Statement of Rights (currently Charter of Rights). Rather than stipulating a signed copy, it is left to the provider to determine how they demonstrate compliance.  
 
This change supports the embedding of rights in the delivery of funded aged care services and removes an administrative ‘tick-box’ requirement.

HCP recipients transition to the new Support at Home Classification

𝗛𝗖𝗣 𝗿𝗲𝗰𝗶𝗽𝗶𝗲𝗻𝘁𝘀 𝘁𝗿𝗮𝗻𝘀𝗶𝘁𝗶𝗼𝗻 𝘁𝗼 𝘁𝗵𝗲 𝗻𝗲𝘄 𝗦𝘂𝗽𝗽𝗼𝗿𝘁 𝗮𝘁 𝗛𝗼𝗺𝗲 𝗖𝗹𝗮𝘀𝘀𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻

Following Digital Transformation Tech Talk on the 2 April 2025 facilitated by the DOHAC please note the following:

All current HCP recipients will transition to four interim transition classifications at the same level of funding they are currently assigned on
Transitioned HCP level 1
Transitioned HCP level 2
Transitioned HCP level 3
Transitioned HCP level 4

The clients in Transitioned HCP levels will retain the same level of funding and ‘no worse of’ principle as described in the Aged Care Act.
Therefore, the clients on Transitioned HCP levels classifications will have quarterly budgets in line with their current level of funding.

When/if the client on Transition HCP classification is re-assessed for a high level of need, they will be transitioned to a relevant Support at Home Ongoing Classification while retaining their “grandparented” no-worse principal conditions as described in the Aged Care Act.

These arrangements will cause minimal disruptions to the clients and providers.

Support at Home Training for providers package is going to be released for the providers in April/May and a full version of the Support at Home Manual V2 will be released in June 2025.

A response from Victoria to Pricing Approach for the Support at Home service list 2025-26

This submission represents the collective insights and perspectives of 70 attendees from 52 diverse organisations across Victoria, following a series of three roundtable discussions held in October 2024. The roundtables, expertly designed and facilitated by Enkindle Consulting, brought together representatives from a wide range of sectors including community health, local councils, not-for-profit organisations, state health services, and Aboriginal community-controlled organisations. These sessions provided a comprehensive view, with active participation from those operating in metropolitan, regional, rural, and remote areas of Victoria.

​This collaborative effort aims to inform and shape the pricing approach for the Support at Home service list for 2025-26, ensuring that the voices and needs of all Victorians are considered.

Strong Victorian Response to the Draft Aged Care Act ​

On behalf of the SSD Connect Alliance, Bayside City Council SSD and Eastern SSD Partnership, a BIG THANK YOU to the aged care leaders who shared their perspectives on the proposed new Act.  

We delivered our written submission to the government in February. You can access the submission here.

The submission is the result of 4 roundtables held in late January 2024.

124 aged care leaders from 75 organisations participated in the roundtables, offering comprehensive representation across metro, regional, rural and remote Victoria. Community health, local councils, not-for-profits, health services and Aboriginal community controlled organisations had a seat at the table.

News in brief - Aged care reform

Serious Incident Response Scheme

As the aged care sector prepares to transition to the new Support at Home program for 1 July 2024 it is important that service providers prepare for the regulatory and program changes that will impact their organisations, staff and clients. 

To help keep you connected to the ongoing changes the Eastern Sector Development Team will host periodic briefing sessions that provide you with a summary of various reform activities, consultations and changes in the pipeline. The first News in brief session was held on the 5 October 2022 and the second on 9 February 2023. The recorded webinars and a summary of key consultations, resources and links provided here.

Following the ESDT online Forum held on 18 October 2022, we are pleased to make available, a recording of the 'Are you ready for SIRS' presentation. This session is based on the Aged Care and Quality Safety Commission Best Practice Guide to Incident Management and provides a great opportunity for all providers to check their understanding of good practice incident management. 

The session focuses on your obligation to have (and maintain) an effective Incident management system (IMS) and outlines the key elements of a best practice approach which includes the establishment of policies and procedures, recording tools/templates, education and communication and good governance and accountability. The session also includes information about what you can be doing to prepare for the introduction of SIRS and provides links to useful resources. 

Specialisation Verification Framework

Navigating the aged care system - The care finder program

The Specialisation Verification Framework has been established in response to recommendation 30 of the Royal Commission into Aged Care Quality and Safety which stated that there should be verification to ensure ‘that the provider has proper grounds for making any representation of being able to provide specialised services for groups of people with diverse backgrounds and life experiences.’

Developed by Australian Healthcare Associates (AHA), the Framework seeks to address this recommendation, by embedding a focus on diversity in the aged care system and helping senior Australians to choose providers that meet their specific needs. Essentially, the framework sets out the conditions under which providers can promote their service to different communities on My Aged Care (for example culturally and linguistically diverse people, LGBTI, Veterans, etc).

This online session will introduce you to the new proposed specialisation verification framework for My Aged Care which commenced on the 27th June 2022. Included in the session are the key features of the framework, the two-tiered specialisation criteria, expectations of the verification and certification processes and discussions on what you can be doing to become a specialised provider.
The aged care system is complex and not everyone is able navigate their way to a successful outcome on their own.   People will and do experience barriers to navigating and accessing the services they need.  To improve access and navigation of aged care, the Department of Health & Ageing is introducing the Care Finder program in January 2023.

This online session will introduce you to the new Care Finder Program and discuss how it links with the broader aged care navigation and service systems including Trusted Indigenous Facilitators and complementary specialist support.   

The session includes information about the program’s objectives and expected outcomes, the Care Finder target audience, the roles and responsibilities of care finders and the referral pathways.
