Initially established in 2010, regional sector support roles, now known as the Eastern Sector Development Team (ESDT), were implemented by the Victorian Department of Health and Human Services to support the Victorian Home and Community Care (HACC) program. The scope of these roles changed over time and currently we support local Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACC PYP) agencies in the eastern metropolitan region of Melbourne. We work together to share good practice that promotes inclusive access and person centred care.
The key objectives of the ESDT are to:
We meet with the regional sector bi-monthly and encourage the involvement of senior management, program leaders, clinicians and program staff from HACC PYP and CHSP funded agencies, service delivery partners and key stakeholders in the region.
In addition to the regional practice forums (see meeting schedule below), we recommend providers build their capacity through other ESDT resources and collaborative opportunities promoted via the ESDT website and YouTube channel.
